Its good to know that you are planning your trip to NA of Pakistan.
Well you can start your journey by first visiting to Kaghan Valley and then after that crossing babusar Pass (4173 m above mean see level) you can enter into the Diameer valley and then to Gilgit and Hunza.
You can get to Naran, from Mansehra via Balakot. From Naran you can go either by Jeep to Babusar Pass and cross it over and reach Chilas. But it all depends on the road condition. I had come to know that Road has now been built to Jalkhat, a place after Burawai. IF road does not permit to go further on jeep, you can leave your jeep behind, walk to a nomad village called Besal (3-4 hours walk from Jalkhat) stay at Besal (Remember no Hotel: you should have your own tent to stay at Besal). Leave early morning around 6:00 AM go cross the Lulusar lake and in 4 hours you would enter a lush green valley of Gittidas. From here you would walk up for Babusar Pass. Depending on your speed & your past trekking experience you will reach the top in 4 hours. Now they have a chai khana on top and a weather forecasting tower (if it has survived the strong winds of the last winters). From top you will descend to Babusar Village (be prepare for severe cold weather, Glaciers and snow fall). Try not to stay long at the top. you may reach Village in 4 hours.
Stay the night in the babusar village.
Remember leave Besal early morning not later then 6:00 AM or you would have to pitch your tent on the descend from Babusar pass.
Next morining you can hire a jeep for Chillas (around couple of hour drive to Chillas).
From Chllas you can go to gilgit (3 hours) and then to Hunza (2 hours). Sakardu is in th different direction and may take 8 hours from Chilas.
